Viewing your Medical Record Online
You can use the NHS App or log in through the NHS website to access your GP health record. This is a record of your personal health information. It may include details about your health conditions and treatments you've had.
There are a few different visibility levels available for patients to view their medical records online. We have tried to simplify this by breaking it down below. For more information about your GP health record please see here.
Sign Up for Online Access
In order to view your medical information online you must first sign up for an online access account and verify your identity using services such as the NHS App.
What Information Can I See?
Once you have logged into your account and verified your ID, you will automatically have access to all information (including letters and free text comments) held on your GP record, after whichever date below is the latest. This currently only applies to patients aged 16 or over.

Historical Access
Information held on your medical record before the above dates is not made available by default, this is sometimes referred to as historical access. To request access to view this information online, please complete this form:
Please note: Historical access does not currently include letters or free text entries. These are currently only visible from the specified date above. If you would like to request access to information that is not visible via historical online access. Please submit a Subject Access Request (SAR) to the practice.
Proxy Access
As a parent, family member or carer, you may be able to add someone to your NHS App, this is sometimes called having a linked profile or having proxy access. To request proxy access, please complete this form:
